Delivery will be the week before Thanksgiving.

Complete the form with a Pen.  Collect the money at the time of the order.  Students: Remember, please only talk to people that you know.

Collection:
~Collect money up front. To help alleviate the extra check processing for our AMAZING bookkeeper, customers should pay in cash or make checks payable to the student’s parent.
~The parent should then make 1 check payable to Durham Middle School. Double check that you have the same amount of money as the order that you are placing.
~Put the check/cash in a school deposit envelope. Label the envelope as “Orchestra Fundraiser” with student name.
~Put the envelope in the Metal Safe in the hallway by the Durham Media Center on or before October 5, 2012.
~Turn the order forms in to your orchestra teacher on or before October 5, 2012. Keep the PINK copy for yourself.
~Yankee Prizes will be received with the product arrival.

Thank you for all you do to help support the orchestra program!
